Real life senarios

If it aint broke, why fix it?
"What 's the point of getting a maintenance chap in to inspect the hoist? I know its getting on a bit, but its never let us down before. We'd only have to put our prices up"

Consider this
An employer was found guilty of breaching health and safety law by failing to maintain a hot water boiler, which exploded, fatally injuring two people, an employee and a contractor. An engineer employed by an insurance company was also convicted, of failing to carry out an examination of a boiler pressure relief valve. The Crown Court fined the employer a total of £150,000 and ordered payment of £125,000 in costs. The engineer was fined £1000 plus costs of £250.
